How Do I Register?

First time users must register and create a User ID and Password prior to utilizing NQBP Connect®. Please click here and complete the registration form.

Once the form is complete, Click "Continue". If others from your company location have already registered you will be asked if you want to use the existing information - Select "Existing" and then "Submit".

You will be directed to the Sign In page. Sign in using your new User ID and password, and you can make a new referral!

How Do I Make a New Referral?

  1. Sign in to NQBP Connect® using your unique User ID and Password.

  2. The Welcome page will be displayed.

  3. Click on "New Referral" in the upper left hand corner of the screen. The referral request form will appear.

  4. Select the case type (WC or Liability).

  5. Select the service(s) that you would like performed.

    If you are unsure which service(s) to select you can leave this section blank and a Service Coordinator will contact you to discuss your needs upon receipt of the new referral, or you can contact NuQuest/Bridge via phone at 866-858-7161, option 2.

  6. Enter as much information as you can in the referral form. Note that at this time, it is only necessary to enter the Claim #, Social Security Number, the claimant's First and Last name and the fax number for the payer listed on the referral. A Service Coordinator will contact you upon receipt of the file material if additional information is needed.

    However, providing as much information as possible will improve turnaround times and will eliminate the need for NuQuest/Bridge Pointe to contact you regarding any missing information.

  7. Select "Submit" at the bottom of the page.

  8. The Thank you page will appear containing a brief summary of the information that was entered as well as detailed instructions regarding the necessary material that NuQuest/Bridge Pointe will need to complete your referral.

    You may print this page for your records by clicking the "Print" button.

    If you have electronic documents, you may upload them directly and securely to NuQuest/Bridge Pointe by clicking "Upload Documents".

  9. You may submit another referral by first clicking on the "Home" button to return to the Welcome page, then clicking on the "New Referral" tab.

  10. If you are finished, click "Sign Out" to log out of NQBP Connect® and return to the Sign In screen.

How Do I Upload Documents?

  1. Sign in to NQBP Connect® using your unique User ID and Password.

  2. The Welcome page will be displayed.

  3. Click on "Upload Documents" link.

  4. Select the first "Browse..." button, navigate to the file that you would like to upload, select the file and click "Open".

  5. If you have additional files to upload select the second "Browse..." button and follow the same steps. You can upload 4 individual files at one time.

  6. Once you have selected the files you would like to upload, select "Begin Upload".

  7. A confirmation will display at the bottom of the screen listing all files that have successfully uploaded. A notice will also be provided if a file does not successfully upload.

  8. You can continue the upload process as many times as you would like.

  9. Be sure that the file type is one of the following: .doc, .xls, .tif, .jpg, .pdf. and that the file size for any one document does not exceed 20MB.

  10. If you experience problems uploading files, or if you receive notice that a file has not uploaded successfully, please contact NuQuest/Bridge Pointe for assistance at 866-858-7161, and select option 2.

*Please note that the Upload Screen is accessible by clicking on the "Upload Document" link anywhere that it appears in NQBP Connect®.

How Do I View the Status of an Existing Referral?

  1. Sign in to NQBP Connect® using your unique User ID and Password.

  2. The Welcome page will be displayed.

  3. Click on "Referral Inquiry" at the top of the screen. The referral Inquiry page will open.

  4. A list of all open referrals you have submitted will be displayed. Choose the referral you'd like to review by clicking on the corresponding date of the referral.

  5. The referral checklist will display, detailing the status and progression of the selected referral.

  6. To check on another referral, click "Referral Inquiry" and you will return to your full list of open referrals.

  7. You may select another referral to review or if you are finished click "Sign Out" to log out of NQBP Connect® and return to the Sign In screen.

How Do I View Or Print Necessary Forms?

  1. Sign in to NQBP Connect® using your unique User ID and Password.

  2. The Welcome page will be displayed.

  3. Click on "Referral Inquiry" at the top of the screen. The referral Inquiry page will open.

  4. A list of all open referrals you have submitted will be displayed. Choose the referral whose forms you'd like to access by clicking on the corresponding date of the referral.

  5. Click on the "Forms" tab at the top of the screen. The Forms page will open.


    To print a form with Claim Specific Information

    a.  Click on a link to a form in the top set of forms.

    b.  The form will appear in pdf format containing the necessary claim information.

    c.  The form can be printed and/or saved from the pdf.

    d.  Continue to click each of the forms that you need.


    To print blank forms

    a.  Click the name of the form you would like to view from the bottom set of forms.

    b.  The form will appear in pdf format.

    c.  The form can be printed and/or saved from the pdf.

How Do I View Completed Reports?

  1. Sign in toNQBP Connect®; using your unique User ID and Password.

  2. The Welcome page will be displayed.

  3. Click on "Referral Inquiry" at the top of the screen. The referral Inquiry page will open.

  4. A list of all open referrals you have submitted will be displayed. Choose the referral for which you would like to review a report by clicking on the corresponding date of the referral.

  5. The referral checklist will open. Select the "Published Reports" tab.

  6. Click on the name of the report that you would like to view.

  7. The report will open in pdf format. Reports can be printed or saved once opened.
